Hello my name is Amanda Selby and I'm completely confused and stuck on the I-134 Form. I have a book here with me that says that I have to get the form notarized and it's also telling me that I'm supposed to have my tax returns. I'm really confused and not quite sure what it is that I need for it and what I need to do. I mean I got the basics of filling it out but the evidence and the notary has thrown me completely. If anybody could just set me straight it would be greatly appreciated. Everybody is so helpful on here and I could really use the help. Thank you so much.

Hmmm, I might advise you ditch the book and do searches on this site. I'm very doubtful on the notary thing, certainly wasn't done on mine.

You do not need to have the I-134 notarized. As far as the tax returns, you should have a copy of the last 3 years, plus a copy of the W-2s for those years (take the originals to the interview of course) or you can get free transcripts from the IRS for the last 3 years (if you get the transcripts the W-2s are not needed).

There are form and process changes that have occurred in the past few years. The I-134 used to have a place at the end for a notary. It was removed and you don't need that any longer. Three tax returns used to be required for Affidavits of Support; it was changed to one. There was old information about notarizing tax returns pre-2008 when we did the K1. We didn't even do that three years ago. Since your interview is London, I'll tell you some London specific information. You need to prove your income with two good proofs. A tax return with W2s is good. A tax transcript is even better all by itself. London doesn't insist on a tax return (or transcript) if you can prove your income with an employer letter or some of your most recent pay stubs or huge amounts of money in the bank or a pension or Social Security income. So pick your two best proofs and you're done. If you prove a salary over the threshhold, then you can skip the assets part of the form. Leave that part blank and skip the bank statements, etc if you've got salary and income tax proofs. They probably won't even look at the extra stuff in London.

And be sure you download and read the instructions for the current I-134 form over what your book says about the form. It's a separate download from the form.
