Has anyone encountered any problem editing and printing this form?

The blanks for items 4c and d, 5, fiance name, date and place have default characters (i.e."undefined", current date) that refuse to be deleted or edited and show up in the printout. I'm not sure if it's my PDF reader which has an issue but I've tried it in three different computers with the same results.

i was having the exact same problem

i ended up downloading the form and then uploading it into https://www.fillanypdf.com/

and thats how i got it to correctly edit and then print

its more work that way but it works nevertheless

Thanks for the advice.

Apparently it's a browser issue. I was using Google Chrome and when I tried on Internet Explorer, it worked fine.

Google chrome is messing up the forms. I've always had problems filling them out and I tried Firefox and IE and everything's ok.
