My husband says that he didn't receive the DS-3025 form or if he did he doesn't know where it is at. All we have is a vaccination card that has been stamped and dated with the type of vaccination listed. What do I do? When would he have reveived this form?

May 2003- I met Carlos in Brazil on a mission trip.

17 Oct. 2005- I-129F sent to TSC.

Take that card to a civil surgeon and have a vaccination supplement completed.

The civil surgeon should be able to use the immunization records that you have if the supplement isn't available. Mine took a look at the worksheet I received from my panel physician medical in Canada, didn't like the way the doctor in Canada had prepared it so filled out my supplement using my original immunization records instead (the same records the panel physician had used as well).

I just got the Tetanus-diptheria shot and my doctor wrote down on her little pad (same pad she writes prescriptions on) the following, with my name on top, the date, and her signature at the bottom:

"Td vaccine given today"

Is this a sufficient form of an "immunization record"?
