Hi again Vj ..

I am now applying for AOS but the only things that makes me feel confused and I don't know what to do about it is the vaccinations record ..while my medical examination for the K3 i didn't have to have this for my medical so now I feel confused and I don't know what to file with my AOS paper concerning my vaccination record and what form I should fill in and send with my AOS papers Also , what to do to get the vaccinations record done I don't have one !!!

I really need help on that

This can be a confusing and uncertain issue. Your problem seems to be a simple one, however. Since you do not have a vaccination sheet from your visa medical exam it seems certain that you will need to get an I-693 Vaccination Supplement from a Civil Surgeon (USCIS-speak for a doctor who they have approved). You can find them via the USCIS web site.

Hi,

I had this problem also, I decided to send a letter explaining I didn't have the vaccination form from my medial in England, however I have a RFE on the way and I think it is about this. I spoke to a civil surgeon here in Fresno and they said as long as I had a copy of my vaccinations from my doctors in England they would do the I-693 for me and I wouldn't have to have the injections again etc.

I would get the I-693 done before sending the package, also call around and ask if a surgeon will do this without performing a full medical - I know some people have had trouble with this.
