I'm trying to go ahead and get packet 3 done before my NOA2 arrives so we can just send it off. I looked under guides and found out everything I need to do. But I need to know what exactly my fiance needs to do. What are all the forms and such that we need to complete packet 3?

What's in Packet 3, and the consulate-specific procedures, can vary a bit from consulate to consulate.

Thanks, I just did that and it was very helpful. I'm now wondering what forms actually need to be sent back after I get the NOA2. I looked all over this website and all over the London Website, but it doesn't actually say what you need to send back before you get the interview. Anyone who has gone through London, Please Help!

firstly, nice name :thumbs:

I am going through the London Embassy so I can give you an indication of whats required.

1. Packet 3 contains the following forms:

DS-230 Part 1 (you complete part 2 at the interview)

DS-156 - this needs to be filled in duplicate but DO NOT SIGN IT

DS-156K

DS-157 (only for male applicants)

Your fiance in england needs to collect the following documents: They arent all applicable, obviously if you havent been adopted you dont need an adoption certificate etc...

Passport (valid for at least 6 months) (REQUIRED)

Long style Birth Certificate (REQUIRED)

Adoption certificate (if applicable)

Deed poll

marriage certificate

divirce certificate

police certificate (go to police station in england and ask for a 'Subject Access' form. Costs £10 I think. Takes 40 days to arrive. (REQUIRED)

Court record

military record

3 Color photographs (these must be US Size) (REQUIRED)

Evidence of support (this is quite in depth so search around for more info on that (REQUIRED)

Medical Examination (this needs to be done before your interview, he could actually go ahead and get it done now. The surgery that performs it is Knightsbridge Doctors in London.) (REQUIRED)

If you want a speedy interview date then I highly recommend getting ALL of this sorted before packet 3 arrives so it can be sent back ASAP.

I noticed that you said you could go ahead and book the medical before receiving packet 3. My fiance called the office of the doctor that does the medicals this afternoon, and was told he could book an appt. a couple days in advance and should wait until he receives Packet 3. However, just from reading this board it seems that it can take over a month or so to get an appt. Does it sound as though the receptionist doesn't know what she's talking about? Also, if he schedules his medical now, (before packet 3 arrives), what paperwork should be bring to the medical? Or is it provided by the doctor's office? Thanks in advance....

I'd get the police certificate sent off for straight away - it took 38 days for mine to come!

Also check that the birth certificate is the long form version as this can take a couple of weeks to get sent!

As James stated, all the forms are sent in packet 3 and they are quite simple so nothing really to worry about.

You could be sorting I-134 and supporting documents as that needs to be handed over at the interview and can take a while to get in order (depending on your circumstances & if you need a co-sponsor).

You don't need to take anything to the medical except a passport photo and your immunization records. Oh, make sure MMR, Tetnus etc are up to date. I've heard others say its a few weeks wait for an appointment....but its taking a couple of months to get an interview date so hopefully that won't be a problem. Maybe get one booked for next month if possible??

One thing you possibly need to book the medical is your case number. This isnt one the US Fiance will have. It starts with LND. They may allow you to book a medical appointment without it however, its worth enquiring.

You need a UK passport photo, your passport, and a completed copy of the USA medical questionaire (I'm not sure if this can be downloaded from the internet or not) This is also something you can ask them over the phone.

I can tell you when I called to book mine, they werent busy at all and could fit me in whenever I desired. Obviously that can change.

Thanks so much for all the information. I just have a couple more questions lol, as always. What is a US sized picture? What is a deed poll? And last, does Patrick need to get the MMR and other vaccinations up to date before going to the medical? We thought the doctor at the medical would do that.

What is a deed poll?
A deed poll is when someone chooses to change there name, say they were born John Smith but wanted it changed to Stephen Jones, they can do that legally, this is called deed poll. A name other than the one they were born with.
What is a US sized picture?
This size picture is a slightly different size on the English version. The Embassy does require the US size photo and most photo shops do them you just have to ask.
does Patrick need to get the MMR and other vaccinations up to date before going to the medical? We thought the doctor at the medical would do that.

This is a mixed bag really, first of all get him to go to his local doctor's and see what he has and has not had done, if he needs updating they will tell him. He also needs to get something in writing from the doctors to say that he has had all that is required. They will charge him at the medical for injections that need doing.

From what I understand he will need the vaccinations for AOS once in the USA, they aren't required for this medical. However, a lot of people opt to get them in the UK (insurance issues, etc) before they get to the US and some get them done at the medical.

Hi I had my medical in London yesterday, so everything is very fresh in my mind. Also I have written a veeeery detailed account of the day here

I also phoned them before receiving my Packet3 and tried to book an appointment but she clearly said that I couldnt book unless I have been contacted by the Embassy. They really need the case number beginning with LND and the medical questionnaire which will be in Packet3, which does NOT appear to be obtainable from US Embassy's website.

About vaccinations, if your fiance has no previous vaccination records (or it has been a long time since he had them) then he MUST get the vaccinations done. This is now a requirement and Knightsbridge Dcotors will charge him for this. If he is between 18-65 years old, he will need MMR and Tetanus. He can also get them from his local GP and according to some people they even got these vaccinations free from their GPs.

Yesterday, I was charged £60.00 altogether for the 2 vaccinations in addition to the cost of the medical :angry:

So its upto your fiance whether he would prefer having the vaccinations done on the day of his medical or in advance from his GP etc. I think they have made it mandatory to get vaccinations done at this stage so that you wont (hopefully) have to get them done again for AOS.
