Has anyone noticed that in the last FY, we noticed a huge double retrogression yet the Annual Report from the Visa Office shows they only issued 20,709 F1 visas as opposed to the numerical limit of 23,400.

That's approx 2,700 folks (!) who could have been fitted in FY10/11. I wonder why that is? Could it be that these figures don't include AOS applicants?

Also worth mentioning is that they have clearly issued more than 7% (1,638) visas to applicants from a single country.

What does section D of the bulletin mean? Is everything still the same in terms of limits?

"These numerical limitations for FY-2012 are as follows:

Worldwide Family-Sponsored preference limit: 226,000

Worldwide Employment-Based preference limit: 144,951

Under INA Section 202(A), the per-country limit is fixed at 7% of the family and employment annual limits. For FY-2012 the per-country limit is 25,967. The dependent area annual limit is 2%, or 7,419. "
