I've been doing a lot of research on the preparation of Form I-693 for a K1 visa holder since my fiancée and I are about to file for her AOS after we get married in a few weeks. I just need someone to verify that the info I have is correct. My fiancée, based on her age, is only required to receive the MMR, Tdap, and Varicella shots. She received the MMR and Tdap in the Philippines, and under Varicella on her form DS-3025, it was marked VH (Varicella history) since she told the doctor she had chicken pox as a child. However, the doctor at St. Luke's Medical told her that she would need to get the shot when she came to the U.S. Is this correct? From what I've read, if the Form DS-3025 is marked VH, then you do NOT need the varicella shot since you already had it when you were younger. I just don't want to make an appt with a CS only to find out that I have to go get the varicella shot and come back another day. Which is correct?

Vaccination Requirements for Adjustment of Status Applicants

All adjustment of status applicants must be assessed for vaccination requirements (Table). The civil surgeon must review all vaccination records presented by the applicant during this process and record the vaccination assessment results on Form I-693, Vaccination Record.

The required vaccines based on the applicant's age at the time of the medical evaluation must be administered. Age-based immunization schedules recommended by ACIP should be followed and are available at: http://www.cdc.gov/vaccines/recs/schedules. For example, if the applicant is 12 months of age at the time of the examination, then he or she is required to receive the following vaccines if he or she had never received them previously:

  • Diphtheria, pertussis, and tetanus (DTP) or diphtheria, tetanus, and acellular pertussis (DTaP) or diphtheria and tetanus (DT)
  • Inactivated poliovirus (IPV)
  • Measles, mumps, and rubella (MMR) or measles, mumps, rubella, and varicella (MMRV)
  • Haemophilus influenzae type B (Hib)
  • Hepatitis A
  • Hepatitis B
  • Varicella (if no history of varicella disease)
  • Pneumococcal conjugate vaccine
  • Influenza (if flu season)

Note: If the applicant receives MMRV, then he or she does not need to receive the single-antigen varicella vaccine.

For example, if the applicant is 35 years of age, then he or she is required to receive tetanus and diphtheria toxoids vaccine (Td) or diphtheria and tetanus toxoids and acellular pertussis antigen vaccine (Tdap), MMR, and varicella (if no history of varicella disease). If the applicant had previously received a dose or doses of a required vaccine but had not completed the series, then the next required dose should be administered according to standard ACIP recommendations.

So as you can see if she had varicella as a child then no she does not need the vaccination.

I had this same issue. I had it as a child (the disease, not the shot). The first civil surgeon I called needed blood test to verify I was immune. I called a different CS and they said all I needed was the verbal confirmation. Went to that CS instead (today!) and he signed off on everything and said I was good. Talk to the office and say that you have had the disease and it is marked on your form, but you don't have the blood test. Call around if they won't do it. Most offices won't give you the shot without a blood test to confirm she has had it already. If you are super worried, she can just get the bloodtest prior to confirm and take that to the CS.

I had similar situation: chicken pox in childhood and no shot on the record. When getting my medical for K1 visa I told this to the doctor and they marked the relevant box. I only needed to confirm it (verbally) again once I went to civil surgeon here - no need for a shot.
