I have one more question. I filled out the form for my fiance and I am going to email it to her. Since I am the one filling out the form can I sign at the bottom (application prepared by) and then email it to her?

Hi

please have look to your Consular Webpage.

as far as i know this form is not valid anymore. Instead of DS-56 you need to fill the new ds-160. It need to be filled on-line and no pen corrections are allowed.

Form ds - 160 replace forms DS-156/157/158

Ukraine uses the DS-156 still, not the DS-160. Please always check with your consulate website instructions to know exactly what your consulate requires:

You also need to print 2 copies of the DS-156.

No, these documents need to be signed by the beneficary. All documents but the I-134 are signed by the beneficiary. Most are signed at the interview as well.

So you could prepare it, scan it and email it, but don't sign it.

There is a place on the form for the person who prepared the document to sign.
