Hi guys,

I received the packate 3 from the US Embassy in Prague with all the documents I need to fill in. In the instruction concerning the affidavit of support it was stated that:

"If you are in the USA, the affidavit may be sworn to or affirmed before an officer of USCIS without the payment of fee, or before a notary public or other officers authorized to administer oaths for general purposes, in which case the official seal or certificate or authority to administer oaths must be affixed.

How you submit the form depends on whether the alien you are sponsoring is in or outside of the IS and what type of application is being submitted. See the instructions provided with the corresponding application for detailed information on how to submit this affidavit of support.

So should it be notarized?

Also I was sent an affidavit of support looking like this:

Is this an old I-134 form? because the one that I got is the same as the link with the OMB exp 04-30-07. My fiancee already sent me an affidavit but the one downloaded from the USCIS website..

Should I tell her to fill in the one that I was sent from the embassy?

Looks like the embassy is trying to empty out their boxes of old forms.

Check with the embassy, just to be sure, but I'm betting they won't require someone to use that form. It's ancient. They should accept the current version of the form.
