I asked about the DS-3025 when I went for my medical here in Toronto, they told me that they automatically give everyone the form filled out.

I`m confused about this as well.

I have the form all filled out by the doctor.

I have all the right vaccinations, I actually needed to get the MMR again and did so the day of my medical, and that was also recorded on the form.

The only ones I don`t have on the form are the shots that were not age appropriate for me and the doctor marked the form with that information.

The lady at the office told me that this is the form I send in with the AOS and I believe them since they do many of these per day.

I`m confused because I keep reading conflicting info on this subject on here.

I`ve read the instructions and it says to use this form and you will be fine. But I`ve also read that things being marked "not age appropriate" is not ok.

I don't get this really, if I NEED these shots then how would I possibly pass my medical?

It doesn't make sense to me.

I don't want to pay an American civil surgeon for something I don't actually need.

So frustrating!! This is the only part of the AOS that I'm completely confused by lol.

Just send the DS-3025 copy. If it's not okay you'll get an RFE which isn't the end of the world because then you can just see a CS, get the transcription done and send it back in. This way you save the time and money doing it if you don't need to.

There are just a couple of shots that are age appropriate and the form saying "incomplete" isn't a big deal. It's only if you're actually missing required shots that it matters and that's where people have had issues.. thinking they're up to date when they're not. But like I said, an RFE isnt the end of the world. You can play it safe and get the I-693 transcription, or just send the DS-3205 and see what happens.

As Vanessa said you can do that but I just want to add that in my country when I went and had my medical I had been to my local doctor to have a serology test done to check on my vaccinations as I did not have any papers from my childhood. I took this to the medical and he looked it over and gave me a flu shot and that was it. He never gave me the DS-3025, it wasn't until I got my packet 3 from consulate that they mentioned I needed to have that for POE. SO I rang the doc and they emailed me a copy. When I AOS'd I still found a CS and had them transcribe my DS-3025 over to the I-693 part 1.2 and 5 only as I had my medical also back home.I didn't want an RFE so they got the the I-693 in a sealed envelope and the DS-3025 I sent both. I still got a RFE as they lost my medical from my POE so to be on the safe side find the CS in your area from USCIS site and get them to transfer it over to the I-693. And make sure you have a copy of your medical from Peru in case they lose your medical papers. I didn't but was able to call AUS and they emailed me a copy.

I have another question about the AOS, I received a CD along with the vaccination form after my medical, so do I need to send that disc in with the I-693? I'm not sure what to do with it, they just said I would need the paper for my AOS.

If you have all the vaccinations required based on gender/age/medical and what not, then send the DS-3025. They can not force you to get a vaccine if it is not age appropriate. I am sure there is a lot of other members who will tell you that they did not take all the vaccines that were on the list for the same reason "age"

You are thinking extremely tooo much into it to the point of a headache im sure sad.gif. Don't get me wrong I was worried too, my husband did not have a single record of any of the vaccines he has received since birth and I did not know what to do until we talked to our doctor.

Ok, I have another question, may seem stupid but I would rather be safe than sorry. I'm going to have my vaccinations transcribed by a civil surgeon just to be on the safe side but do I do it in maiden name or married name? My medical info is in my maiden name but I've changed my name since getting married. Sorry if this has been answered before.

If you "officially" changed your last name when you got married then you need to use your new last name, just make to take the necessary paperwork (passport, marriage certificate, etc..) when going to the CS.

In my case all my forms except for the I-693 are in my married name. I saw the civil surgeon before getting married so it was in my maiden name.. I honestly don't see a problem with it. It would be nicer though to have them all in the same name.

My Journey:

We met through a study-abroad program in Shanghai, China in August of 2009

We got engaged March of 2010

I received my K1 VISA in 6 months (June-December 2010)

We were married 04/02/2011
I received my conditional 2-year greencard (AOS) in 2.5 months with no interview (April-June 2011)

Our son was born 02/03/2013

I received my masters degree in Speech-Language Pathology 04/17/2013

I received my 10-year greencard (ROC) in 3 months with no interview (March-June 2013)

My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)

I'm still confused about all of this. The dr. in Peru didn't give my husband any official immigration papers, just a doctor's paper with what vaccine's he gave him.

I'm pretty sure his vaccines are complete. The thing that's confusing me now is the TB test. They didn't do a TB skin test, just a chest x-ray. If my husband goes to just get his vaccines transcribed onto the i-693, should he get a TB skin test from a local clinic before that and bring proof of that too for the dr. to transcribe? Or just not worry about the TB skin test until he maybe gets an RFE?

I found a dr. that does a whole physical for $110 which seems pretty reasonable to me or a dr. that transcribes the records for $60. I'm kind of leaning towards spending the $110 just to be on the safe side although I'd really like to avoid it?

If I were you, I'd ask the doctor who transcribes if he will transcribe from the doctor's records (as opposed to requiring the DS-3025 for transcription.) My understanding is that the chest X-ray is sufficient for TB testing. Also, when we went to get my spouse's DS-3025 transcribed, they found we were missing a required vaccination, which they administered there (for an additional fee). We also had the option to go somewhere else, get the vaccine administered, and bring documentation back to the civil surgeon who would then add it to the I-693. So I'm glad we went to a civil surgeon and covered our bases.

In terms of doing a full medical, it shouldn't be necessary unless USCIS lost his overseas medical report, or if he had any Class A medical conditions. So it's up to you. How big a deal would an RFE be to you guys? It could delay his green card, maybe even his AP or EAD (although from what I see here, those will often go through independently even if there's an RFE for the I-485).

We did not receive a copy of the DS-3025, even though we repeatedly asked the medical clinic and the consulate in Ghana for a copy. The consulate said we were not authorized to have copies of those forms (which is not true, as the instructions of the form clearly state that we SHOULD be given a copy). We asked a helpful immigration officer at our POE to give us a photocopy of the DS-3025 for our AOS, and he actually included the full medical exam, so we knew what was in it and submitted a photocopy of the full exam in our AOS packet, plus the transcribed I-693 vaccination supplement.

Hi LoveInPeru,

In order to avoid any possible RFE my husband paid $130 to get the exam, TB test, blood test, and have the vaccines transcribed.

Take note, My husband had his immigration medical exam in Mexican, and was not given anything. Remeber, in order to obtain K-1 visa you do not need to have vaccines only medical exam. So we knew he will need vaccines here in US in order to adjust status.

Before looking for CS my husband went to see our insurance doctor to take all the vaccines required to make sure we would not pay extra at with the Civil Surgeon.

I understand the concern since we have invested a lot of money to get green card, but you are thinking tooo much into the situation and making it more difficult for yourself and partner.

The chest x-ray was the TB test. You do not need another exam or another TB test. You only need to complete the vaccines. The civil surgeon can look at your vaccination records and let you know if you are missing any you need to get still. You fill out part one (personal info) of the I-693, the civil surgeon fills out part 2-(Vaccinations section only), and part 5 (where he signs it). That is all you need for adjusting from a K-1, since the medical was done within the year.
