Do I need anything notarized? (time running out until interview)

I stumbled across CDJ-402, which states:

"I hereby certify that I am legally free to marry and intent to marry __________________"

Then there is a spot for a signature and a consulate signature and stamp.

This doesn't have be filled out by me before he goes? Does it????? I don't think that it does and have never heard of this form before finding it in the huge pile of papers that I have printed for my fiancé's visa. Has anyone else heard of the CDJ-402?

I am so worried about forgetting to put something in his packet.

Right now, the only thing that we have notarized is an affidavit of support from our co-sponsor.

You don't need anything notarized. This is a sort of updated letter of intent, as far as I know consular officer didn't ask for it at interview( that from petitioner). I think it's not bad idea that your fiancé brings yours, just in case, too much is better than not enough.

Only the visa applicant needs that form. It is an updated intent to marry. They just want the applicant to once again state that they intend to marry you when they enter the US. Don't have him sign it though. He will sign it in front of them there.

That is how it was for my fiancee. Don't worry about notarization.
