So I decided not to chance it and made an appointment with a civil surgeon in Pittsburgh for my i-693A (Smithfield Medical Centre - $25 for any of those needing one).

Our appointment is at 1:15pm which suited fine as Dave will be able to take me before work.

They've just announced on the news the funeral procession for the mayor, and it looks like most of the roads in the area downtown will be closed from 10am and parking is limited!

We're going to leave 1½ hrs before the apointment (30 min drive usually) and prepare for a long walk!

Just to play it safe I recommend taking the xray from the physical along with you. The CS that we went to wanted to do an xray so I whipped out the one from Claudeth's physical! Good luck

Found it! But I just thought of something...

Does the i-693A have to be done in my maiden name to match my medical or doesn't it matter?

Any advice appreciated.
