Ok,

I earn 3 to 4 times the poverty limit. My Ukranian Fiancee holds an original and 2 copies of my bi weekly paycheck itemizations showing year to date pay, starting date of employment (1997), deductions (savings...), and biweekly pay. She also holds copies of 3 years of tax returns.

Does she need the I-134 in addition to the above for the interview?

The embassy states:

"Petitioners may submit a notarized Affidavit of Support form I-134 if they wish."

Where do I find the official I-134 form?

Does she need a copy of my divorce decree?

According to the info found on Kiev's embassy website:

Your fiancé(e) will be asked to present:

* Valid passports or travel documents for the beneficiary and any dependent children

* A notarized statement from an absent parent giving permission to a child to immigrate to the US along with a photocopy of the photo and signature pages of his/her passport OR court decree of sole legal custody OR death certificate

* Birth certificates for the beneficiary and any dependent children

* Proof of termination of any prior marriages of both petitioner and beneficiary

* Change of name certificates or marriage certificates

* Police certificate(s) for the beneficiary and any dependent children over 16 years of age

* Medical exam results for the beneficiary and any dependent children

* Proof of adequate financial support once in the United States to ensure that your fiancé(e) and dependent children will not become public charges

* Supporting documentation verifying the relationship between the petitioner and beneficiary

It's up to the consulate to request an I-134 or not. As per the instructions on their website, it's not a requirement there, so you're free to submit it or not, provided that you do submit proof that you can support your fiance in the U.S.

As far as I know, copies of all divorce decrees that you and your fiance have (not just the latest one).

Wait. I just re-read the instructions and they don't say "copies", they say 'proof'. So it's best to check with the embassy whether to send them a copy of an original decree. Better be on the safe side, right?

Whether or not I-134 will be required is at the discretion of the consular officer processing the case. They will exercise the discretion, or not, after examining the financial evidence that you provide.

So I guess what Yodrak said means that even if I-134 is not a requirement, it's a good idea to have it ready for the interview, along with the supporting documents.
