i have recived a RFE for my AOS application recently stating:

A review of the records indicates that you entered the United states pursuant to a valid k-1 visa and all USCIS records relating to you have been reviewed, but the U.S. department of state medical forms DS-2053, DS-3024, DS-3025 and DS-3026 are not in any of those records. therfore a new medical form must be filled out by a civil surgeon and sent to the USCIS.

As a result of that, i have booked an appointment at a local approved civil surgeon for 2moro. my Questions are, Does this mean they have lost my medical forms from my medical in the U.K.??? and are these listed forms things i need to provide to the civil surgeon or are they forms he will have and fill out? all i had to bring to my uk medical were photos! ive never heard of these forms! any experience or advice is very much appreciated :thumbs:

Sounds like they lost it :( At your appointment tomorrow, the civil surgeon will fill out a form I-693. I would call and ask beforehand if they have those forms on hand, and if not, then maybe you could print one out (they should have it, but just in case). The Civil Surgeon will give it back to you in a sealed envelope. Don't open the envelope and send it back to USCIS! Good luck!

You could try to call Knightsbridge and see if they will give you a copy.

for anyone interested. i had to have a new medical with a civil surgeon. who scraped all the old DS forms and just filled out one replacement I(6 something) form.

yeah the CS's in the US use the I-693 form. The other forms are for the panel physicians out-of-country.
