While this topic is still fresh, I wanted to share yet another person who got their greencard approved this week without having their immunizations transcribed to an I-693 by a civil surgeon.

I just got this message

Perfect. Nick did the same. He didn't want to spend £25. Then his GP said DTP was in short supply at the time and reserved for babies. A couple of days later, the nurse called and said we have DTP with your name on it. When do you want to come in? Yay! So it all worked out. The immunization nurse at Knightsbridge was super helpful to him. She mailed a copy of his DS-3025 form to him after it was signed off. When you get to AOS, you're going to read a lot of people saying (and VJ guidelines) to pay a civil surgeon to copy your DS-3025 shots to an I-693. I can tell you about 100 people who didn't because they got it complete at their visa medical. A photocopy of your DS-3025 with Tdap/Td/DTP etc, MMR, and VH (by varicella)on it; the "results" marked incomplete-may be eligible for blanket waiver; and the doctor's signature will be considered acceptable.

What do you mean by blanket waiver?

My immunisations are up to date, but at the medical it was marked incomplete. The nurse said that it's because some of my immunisations were only good for 10 years and they will run out in a couple. So do I need to get mine transcribed or what?! I'm so confused by this.

My immunisations are up to date, but at the medical it was marked incomplete. The nurse said that it's because some of my immunisations were only good for 10 years and they will run out in a couple. So do I need to get mine transcribed or what?! I'm so confused by this.

Look at your sheet. In results there are several choices. There is a box for "incomplete" which is probably ticked. Below that is another box saying something about the applicant may be eligible for a blanket waiver which also should be ticked. I don't know if the forms are still identical, but somewhere in results is a place to the doctor to indicate you may be eligible for a blanket waiver.

I've written about this so much that I'm just going to put some links to a couple of previous posts where I explained blanket waiver if you'll please look over those. I don't have time to answer because I need to call the cable company and argue with them right now.

The Swedish vaccinations nurse told me not to worry that it was marked incomplete, that it was just because tetanus has to be boosted.

Well, I went to my GP nurse and she gave me an MMR (she couldn't find my original child-dose of MMR) and it has been 12 years since I had a Tdap, so she gave me one of those too. She printed me out a record of the "1st MMR" dose and the Tdap which I scanned and emailed to Bentinck Mansions.

Had an email from one of the girls in admin saying they'd printed it out and given it to the Vaccinations nurse, and she would contact me directly if there were any problems - this was on... Wednesday, and I haven't heard anything yet. I will follow it up on Tuesday with a phonecall (stupid public holidays) and double check.

Hopefully Katie's I-134s will arrive sometime soon. I am resisting sending in the DS-2001 until I have the I-134 forms in my hand.
