I was wondering what forms are required to be sent in packet 3, how many copies etc for the London Embassy

What about photographs, the I-134, and other supporting documents?

Everything you need is right here http://london.usembassy.gov/iv_15.html That is your packet 3.

Forms are in the right column.

Don't overlook the link in Step Two for the documents to bring to the interview which must be in hand to send back the DS2001.

Links on the left you need to click are Medical, Interview, Fees.

On any page, don't overlook the right column. Some say "...for additional information." Click it. Read every page and link. No skimming or you may miss some details. There a 10 page (or so) document on the medical. All important stuff to look at. Take notes. There will be a final exam. :P

On DS156 do I leave state/province empty, or put none. As we dont have states or provinces but counties. If it means counties, what county is London for the passport issuance.

Question 29 - Would that include multiple visits for a religious visa? Would I need to put each date?

You don't have a state or province- None

List every time you've been to the US, even to Disneyworld when you were 3 years old. That box expands as you fill in dates. My husband had so many I think his looked something like:

June 1985-6 days; Apr 5 1995-15 days; Sept 18 1995-7 days; Oct 22 2001-21 days; etc

Notice when he didn't know an exact date, he just put the month and guessed the number of days. He figured out some by trying to read blurry passport stamps and finding flight itineraries in old emails. I think he maxed out the box, so went for that shortcut version trying to minimize characters and spaces as much as possible. There was no problem with how he did it, meaning it was never questioned or rejected.
