Ok we got our NOA2 last week and now I suppose I need to send the packet 3 into the NVC or Colombia Embassy?

1. Does the DS-230 get filled out with my fiance information?

2. DS-3032 is choice of agent which can be e-mail from what I have read with some success

3. Where is the DS-2100?

I can not find that form anywhere, I have searched high and low it seems to not exist on NVC site or the Embassy site and not able to find anything here on VJ either, but packet 3 has it listed and also noticed that packet three is from 2010 is the latest I can find

Does anyone have a sample cover letter sent with packet 3 and can it be e-mail I saw a couple of post that said it can be e-mailed

What needs to be signed, if I am the choice of Agent?

Thanks

Ok I just managed to find the DS2100 under the immigrant visa section on the embassy page but if you can answer any of the other questions I appreciate it
