Hello,

My wife is trying to file for adjustment of status in the next few days. I thought we had collected everything we need but then I was reading through the guide and saw something called an I-693. Is this required for K3 applicants. My wife had an extensive medical examination in Cameroon for her K3 visa but the embassy took all the paperwork. Do we still need to complete the I-693?

Hello,

Are all adjustment of status applicants required to have a medical examination?

Most applicants for adjustment of status are required to have a medical examination. The medical examination must be conducted by a civil surgeon who has been designated by the USCIS. The examination must be documented using Form I-693, Medical Examination of Aliens Seeking Adjustment of Status.

EXCEPTIONS:

2. Ks: If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon.

Hello,

Thanks for the replies. My wife is on a K3 visa and had her medical in January of this year. One of the replies stated:

"You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon"

Has anyone completed a "Vaccination Supplement." What does this include? Where do I get the paperwork to complete this? Or do I just contact a Civil Surgeon and tell them we need the vaccination supplement completed?

Your Vaccination Suppliment needs to be completed by a Civil Surgeon.

See the various other threads on the subject, as fees and experiences vary. We paid $160 for ours, incl. a TB test and add'l vaccinations, and that's getting off lightly, though some people paid $25 for just the form.

I also entered the country on a K-3 visa. You will need the I-693A immunisation form to be filled out by a civil surgeon here. You can take along a copy of the immunisation work sheet that was completed in the original country and if all vaccinations are completed then the civil surgeon will simply fill in a I-693A which must be sealed and stamped along the seal by the surgeons office. We paid $70 to have this done as all immunisations were completed and a medical was not required but I have read on here that the amount you pay depends on the office so it is worth calling around if you have time to do so.

The civil surgeon will have the form. Some doctors will want to do the entire physical again. Find one that will just do the supplement. Take your vaccination worksheet with you (if you have it).

Hello,

Thanks for the replies. THe embassy took all of our records and we didn't think about photocopying them (actually I believe they were sealed). I just located a civil surgeon in our area so we will go sometime next week but I believe we will end up having to do all the vaccinations again. Thanks for the information.

If you don't have any documentation stating that you have had the vaccinations then you can have a blood test I believe to see that they have been given rather than having them all again which I am not sure would be good for you.

Good to know that Pras will NOT need a new examination (she had one completed in Dec 2005, and we have an exam appointment for 9/15).

I'm going to print out the page to take with me to the Dr., and then do vaccinations-only.

(dunno exactly how much that will run).

why don't you call them first and ask how much its gonna cost ya !!! cuz some civil surgeon's would just rip-u-off... and i m not joking. I had called few myself..... everyone charges different. The cheapest one i found, would do it in $110.

I wanna thank you for providing me the doctor's number. I called them today and scheduled an appointment for my husband to get his vaccinations done on this thrusday. and they said they would charge $70. Which is cheaper than the one before. saved $40.
