Help about form D156

Hi,

I have a doubt about the form DS 156 so here it is:

*Have You Ever Been Issued a U.S. Visa? My answer is yes

Then it asks: Enter Additional Visa Issuances Here: Should I put there about my visa Extension (it is a DS paper) or I don't need to put it there. I need an answer assap. So just answer me yes or no, because I have no idea.

* I think no is the answer because an extension is an extesnion of one already issued visa. Am I right about it?

If you have had a visa before then YES is the answer

About the Visa I know I need to put yes. My doubt is about the extension (DS) should I put it there as well. Or an extesion doens't count because is an extension of an already issued visa.

I hope someone knows the answer about this question,

I would not worry about the extension part darl you have put the visa and yeah the extension is just that a part of the already visa
