Veronica's DS 3025 does not say incomplete on it.She has been here from Colombia for just a bit more than a month.I paid for all the shots in Medellin. We have her DS 3025. Do I take a I 693 to a Civil Surgeon with the top completed so he can fill out the bottom and seal it or do I do nothing with the I 693 and DS 3025? I'm working on getting AOS paper together. What I read on here was I don't have to do anything with this unless it's incomplete.

Look there has been many debates over this issue and if it is complete then all you have to do is send the DS-3025 in with AOS but on the other hand some have received RFE's and needed to have their 3025 transcribed over to the I-693 part 1,2 and 5 vaccination part only. SOme have had no problem with sending just the 3025 and others have but most go to the CS and get the 3025 transcribed over to the 693 and the doc will seal it in an envelope and you send that with your AOS. I sent that and also a copy of the 3025 with mine. I was unlucky though as they lost my medical as well and they wanted me to have blood and Tb tests done again mmmm I was fortunate I was able to get a copy of my medical from my country and I sent that back to them along with the results of my tests and another copy of the 3025. Totally up to you.

We sent our AOS papers in February and only included a copy of the vaccination form from the doctor in Bogota. A close reading of the instructions, at least in my opinion, does not require anything from a doctor...unless it's been more than a year since the physical. This is based on a K1 Visa.

No the answer to that is you don't need another medical if it is less than 1 year but the vaccination 3025 over to the 693 is not another medical it is just a doctor filling in a form.

Here's what the instructions for the I485 say regarding a Fiancee K1 or K2:

8. Fiance(e)s

If you are a K-1 fiance(e) or K-2 dependent who had a

medical examination within the past year as required for

the nonimmigrant fiance(e) visa, you ONLY need to submit a

vaccination supplement, NOT the entire medical report. You

may include the vaccination supplement with your Form

I-485.

I inserted CAPS on "only" and "not."

Yes I have read this before when I went through AOS and yes you do only have to supply a DS 3025 vaccination supplement and no one has the medical anyways as USCIS have it from when you enter USA. It is just suggested to have the 3025 transferred over to a I-693 by a CS to prevent a RFE is all I am saying .It is purely a personal choice as well as the instructions I do know this.

Ok I don't need to but just to play it safe i'm going to pay the CS to fill out the bottom of the I 693 and seal it. Thanks

And wise choice just part 1.2 and 5 ok

I paid only $25 to have the doctor transcribe it to the 693. Totally worth it, if you ask me, saves me the worry of wondering if I should have done it or not. It's really your call :)

My Journey:

We met through a study-abroad program in Shanghai, China in August of 2009

We got engaged March of 2010

I received my K1 VISA in 6 months (June-December 2010)

We were married 04/02/2011
I received my conditional 2-year greencard (AOS) in 2.5 months with no interview (April-June 2011)

Our son was born 02/03/2013

I received my masters degree in Speech-Language Pathology 04/17/2013

I received my 10-year greencard (ROC) in 3 months with no interview (March-June 2013)

My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)

Of course you could pay to get the vaccines transcribed, and then pay for it all over again when you get an RFE for a lost medical like us. $50 for the transcription we could have saved had we known we'd get an RFE for the medical and have to pay $230 for the whole thing again anyway. :huh:

But it's just luck of the draw really.

We followed the K-1 instructions to the letter and only sent the DS-3025. A lot of people adjusting from K-1 did that also without problem. Some weren't so lucky for whatever reason. I suppose the worst case is that we have to address any RFE if, and when, we get one. What's another two weeks after so much waiting? Additionally, who is to say there won't be another (silly or unforeseen) reason for an RFE?
