How to write down my name on forms???

Hi :P

This might be a silly question but I'm stuck here :huh: How should I put my names on forms? my maiden name is Patricia Andrea Miranda Correa, Im taking my husband's last name so it will be now Patricia Andrea Miranda Edwards ( he want me to keep 4 names just like him) so my question is, where to put all this???

Is it correct if I do this:

first name: Patricia Andrea

Middle name: Miranda

Last name: Edwards

or

first name: Patricia

Middle name: Andrea Miranda

Last name: Edwards

silly silly but please help meeeee!!!!

Your last name would be correa not edwards since it asks for your maiden name(meaning your full name when you are still single

This is incorrect. She is asking about filing out AOS paperwork for after they are married.

OP - There is no real "right way" to do it, but if you want an opinion, I would put one first name, and two middle names, then your new last name. Good luck!

hi, you are right harpa, I just overlooked at the statement, i thought it asks for maiden name, Sooo sorry...my apology!!!!

Sorry i thought i thought it ask for your maiden name... Your middle name would be correa and Edwards for your last name

This is only the convention in the Philippines, not everywhere else. She can have her name as she likes.

(nothing against you, just want clear information out here, hope you understand)

I just put my former last name as my maiden name and wrote my husbands name as my surname :)

My Journey:

We met through a study-abroad program in Shanghai, China in August of 2009

We got engaged March of 2010

I received my K1 VISA in 6 months (June-December 2010)

We were married 04/02/2011
I received my conditional 2-year greencard (AOS) in 2.5 months with no interview (April-June 2011)

Our son was born 02/03/2013

I received my masters degree in Speech-Language Pathology 04/17/2013

I received my 10-year greencard (ROC) in 3 months with no interview (March-June 2013)

My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)
