It seems like I have everything I need but for one thing -- I have been looking around but just couldn't see where to get it. In the I-751 instructions, under the Evidences of Relationshiop, this is what's stated:

So, question, is there a specific affidavit form that they have to fill out or it can be a freehand letter?

Keep it to one page, just the basic facts.

I affirm under the penalties for perjury that the above-listed facts are true to the best of my knowledge.

i filed for ROC last february but i did not submit affidavits because they are optional..

but if u want to submit them, there's no required or specific format for this.. gluck! :)

If you have sufficient other evidence that proves how you have co-mingled your financial, personal, and social lives together, you generally can get by without submitting affidavits. I didn't submit any and had no problems getting my conditions removed. If you do decide to have them though, basically, the individuals needs to identify themselves fully, provide complete contact information, state how long and through what means they know you and any other evidence they have that supports your position of having a valid marriage. This should then be signed in front of a notary who witnesses the signature, verifies their identity and stamps the letter.

Thanks for all the responses.

I have everything except this one -- bank/credit card accounts, car/house title, insurances, etc.

We knew a lot that can testify to our bona fide relationship but with this being optional, I'll guess I'm not gonna bother them then.
