Parent Birth Certificate and Misc. Fees Questions..

Hello guys, I hope you all have a good weekend. I have a little question about my mother's birth certificate. She applied for a late registration and her name does not have an middle name but her name in my birth certificate has. I wonder I I can just submit it like this or we have to do something?

I know in the application, I have to send a $420 check, do you guys know what else are the miscellaneous fees, like when it goes to NVC? Do I have to send more money? How about the St. Lukes? How much would it be?

I remember my friend petition his faher in 2010.You both have the same/similar situation.On his birth certificate his father has a middle name,but no middle name in the birth certificate of his father.But he was good...was approved...
