Name on Visa/Passport

2 questions:

1.) WHen applying for the IR1 visa, does my wife need to put a new name with my family name? If so does she need to update her passport to reflect the same to return to the Philippines when she does/wants to

2.) Kind of goes with #1, does she need to update her passport before/after the visa application process

OR

Does she leave her current name on both and get her name changed when she arrives in states, and then what? Update both passport and visa again?

Family name is the name she is born with: in the US it's the Maiden Name.

Whether you and her change the passport is a personal decision. If she is going to take your name then place that on the original form (I-130) and note it on the g-325 as names used. Chances are the maiden name will still be used but make sure at POE you request a name change or it will cost close to 500 dollars to change the card. My wife did hers at POE Atlanta and it has been smooth sailing.
