Moving to NVC forum, DS-260 is a NVC or Consulate forum topic.

What country are we dealing with, Your profile lacks any details that may help provide an accurate answer. http://www.visajourney.com/forums/index.php?app=core&module=usercp&tab=members

Some countries Police reports are un-obtainable, others, the US consulate makes the request, some "Places" = Cities, and still others, one report covers whole country.

For Cr and Ir visas yes, you must go back to age 16. Not sure about other visas
