Yesterday my fiancee in Moscow had her Medical exam. She did all her immunizations prior to the exam at her local clinic and got an immunization record. When she cam for medical they asked "if she wants to translate her record". She did not completely understand what they mean and she said "no". They said that this is not necessary for K1 visa and did not even ask for the record. They gave her sealed envelope the same day and let her go. Does this mean that she will have to do immunization again for AOS? Should she go and get notarized translation of the record to bring with her? Can she just bring record to US?

Get a translation of record to bring to the USA, It will need to be transfered to an I-693 to be added to the Adjustment of Status application.

Does the translation really need to be notarized? or just a simple translation without notarization or apostile? Just curious

This may be up to the US Civil surgeon doing the transcription to the I-693.

Technically USCIS does not need it to be notarized. Just simply certified to be an accurate translation by the person doing the translation, this means they note they are competent and fluent in both languages on the translation and simply sign and date it.

No notarization required. Obtaining a copy of the 3025 form is the best, but that can only be obtained from the medical agency. You CAN call them and request it after the medical date.

Ok, I am confused now... What about the vaccination records for the CR1/IR1 visas? Do they need to be translated as well? Every time I think I got it all figured out, something else pops up

Nope, in CR-1/IR-1 cases this is a requirement of the visa, so will be handled at the medical for the visa, and be included in the medical documentation. The board certified doctor overseas can handle documentation in the local language just fine.

K-1 is different than CR-1/IR-1

Did she get a copy of the DS-3025 which is the form filled out at the medical that lists all immunizations that she has had? She should have it or get it. Several of us were able to use this successfully in place of the I-693 when filing for AOS. In our case it saved us from needing to visit the civil surgeon here in the US.

Sorry, but I want to be sure my fiancee asks for the right documents....she should ask for a translation of I-693 and ask for a copy of the document 3025? and should this be translated also? Sorry, but just a bit confused....if she asks for the 3025, will she receive it at the medical office that day?...she want to return to Petersburg after the interview and receive the visa and leave after it is in her hand and doesn't want to wait for any more crazy things.....thanks for any help

Have her call and ask to have them get a copy of the 3025 ready for her to pick up at the medical facility. This will be needed for the AOS and for use by the CS to transcribe the records onto the I-693 document.

To the other poster.....it is highly recommended that you send in BOTH the 3025 AND the 693 with transcription of that by the CS for your AOS filing. Some have sent in only the 3025 and been approved and many others have pulled an RFE and had to get the 693. By my count it runs 50-50. We sent in both and had no RFEs.
