I think first you have to start with the airlines that serve your route and just call them up.

It may be hard because all companies have "automated phone service."

This is an area that may vary greatly, but I think the ground rule they may have is that

the kid is "potty proficient." I don't know of any flight attendants that want to change diapers.

No regulations, up to the airline. Page 3 of the link says US airlines will not allow a child under 5 to travel alone; under 8 only on direct/through flights (no connections).

On Philippine Airlines, the minimum age is 8 years old.

@ Lisa C: for 16 hours Manila to Honolulu. read somewhere 4 years old are allowed to fly but for regional only.

@ Rin & John: might need to call airlines if they do special cases
