It is just a simple document where you list the dates and any important event(s) taken place on those dates. For example:

01 JAN 2014

I met my fiancee on xxxxxx and sent her email. We exchanged emails for the next 999 days...

02 FEB 2014

We exchanged phone numbers blah, blah, blah...

03 MAR 2014

I called my fiancee for the first time. blah blah blah...

so on and so forth. Make sure that you date/sign it ahd have it notarized. Make a couple of copies (for the interview and record keeping). Don't forget to include the following paragraph (or something similar to this) in the timeline, preferrably right above your signature:

I, <name>, hereby swear that the information provided in this Relationship Timeline is true and accurate to the best of my knowledge. I shall take full legal responsibility under the law for any fraudulent or inaccurate information listed herein.
