I think I've made a huge mistake. When I went for my medical, the woman asked if I wanted to get an injection there that would complete my vaccination record, or I could just get it at my local doctor and show evidence of it later.

I opted for the second option, and now reading through the AOS guides it says I need to get some kind of authorisation from a civil doctor in the US. I'm so mad. If I'd known that I would have done it there and then in London.

Has anyone else had to do this, that can help me out with knowing what to actually do. What does it mean by civil doctor?

All K1 visa holders have to get a civil surgeon in the US to fill out the I-693 when they file for their AOS. I got my vaccination at Knightbridge and the civil surgeon here just looked at that evidence and signed the form. Just make sure you get whatever you need to have done in the UK and get evidence. It's expensive to have them done here in the US.

you just need to have your vaccination records transcribed by a Civil Surgeon.

Correction... SOME K1 holders need to see a civil surgeon to have their vaccinations transcribed. If the shots are completed a transcription isn't REQUIRED but I got one anyway to try and avoid an RFE (my shots were complete).

OP - in your case where you are missing shots you will need to have the missing shot, then take the proof of that as well as your DS-3025 to a civil surgeon (Sunandmoon posted the link) to get your shots transcribed. Should only be around $50 for the transcription. You can also opt to see the CS and have THEM do the shots in case you're still missing shots that you weren't told about.

Yes. And if you read the instructions here: http://www.uscis.gov/files/form/i-693instr.pdf page 4, Point 2, c, b you'll see it says "if the DS-3025 was not properly completed...". I personally had complete shots but the medical was being lost regularly so I opted to get the transcription to hope that changed anything. Many people have had their DS-3025 complete and not sent the I-693 and had no issues.
