I just moved and need to change my address with USCIS. Which form do I file? I filed for Removal of Conditions in December.

A search of the USCIS website will get you to the AR-11 which can be filed online.

If your I-864 sponsor has moved they would file the I-865 as well. This is filed through snail mail.

Where it says, "Last entry into the U.S." Would I put the date of re-entry from my vacation to Canada last August or when I moved here?

Should have no affect on your case - I do not believe your case will be moved from CSC to VSC for processing - the only thing that would change is the local office, should an interview be scheduled (which you would want to happen since the local office in Missouri would be troublesome to attend - should one be scheduled).

Short answer is 'no', shouldn't slow things down.

It may possibly slow things down. The OP moved accross state lines and therefore the file has to be transfered to the correct office that will complete the application. And we all know how "efficient" USCIS is. Hopefully the transfer will be done smoothly so not to hold things back.
