I-693: Vaccinations Updated in UK but Not Included in Medical

We are just beginning to assemble our adjustment of status forms and had a question about the I-693. My fiance completed his medical exam in December per the K-1 application. He updated his vaccinations AFTER the medical (he was missing a few and decided to get them sorted in the UK so it would be cheaper), so it wasn't included in his sealed envelope. He has all of the documentation from the vaccinations.

Does this mean we have to fill out the I-693 and see a civil surgeon?

Yes, because they weren't shown on a properly completed DS-3025.

Ugh, does this also mean he has to get a physical again? We already went through this...

Does it cost a fortune to see a civil surgeon in the US? He's not yet on my insurance since we're not married until mid-April. Does the surgeon have to perform another exam or is it just to look at his vaccination paperwork and sign off on it?

Also what is a DS-3025? I keep reading about it but my fiance says he didn't get one of those forms. Was it included in the sealed envelope?

The DS-3025 is the Vaccination Documentation Worksheet that you get given at Knightsbridge when you have completed your medical and go to pay. Not sure whether or not he would have received one, if he didn't have his vaccinations done there and then. Hopefully Nich-Nick will enlighten you.

When i went to see a civil surgeon it cost me $100 just for them to put a signature onto the paperwork. No you will not be required to do another medical.

If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

We first met online 2004, but things started to get serious in Dec 2007. Made my first visit to the USA in march 2008 for 2 weeks. Then returned again in Aug 2008 for 3 months.. Then i came back to the UK for 5 months to return back to the USA April 2009 for another 3months.

Remember that the Civil Surgeon is the only who can sign the form I-693

Our civil surgeon took less than five minutes during the whole interview/exam.....so no worries, it is nothing like the medical exam for the consulate.

Seriously!! Try to read instructions. That question is answered clearly in the reference already given you.

Download the I-693 instructions. Read Section III, 2, a & b on page 4.

Sorry man, I'm at work and I can't read through millions of pages of confusing government instructions at the minute (which, for your information, I've already printed out and highlighted so I could ask questions on here for the parts I'm uncertain about). I'm sorry if answering one question was so difficult for you. There are a LOT of pages of instructions for the AOS process and it's not as obvious as you make it out to be. You don't have to reply if I'm bugging you.

I've done nothing but research this ####### for 8 months so please don't act like I'm simply being lazy. I appreciate your help but you don't have to be a jerk about it.

Also don't act like the instructions are the law for this process. I can't tell you how many times I've read something in the instructions or on the embassy websites that have said one thing, but required something completely different. If I went solely on this information and not asked redundant questions on VJ to clarify, we'd have had several RFEs by now.

I believe he should be able to get a vaccine transcription for around $30 on the I-693. He doesn't need a new medical. Either way, CS costs are not covered by insurance. Good luck.

He will have been given a copy of his DS-3025, at the same time as his receipt from the medical in London. Take it and/ or any complete vaccination history to a Civil Surgeon in your area and have them complete the I-693 vaccination section only.

There's a thread called something like 'Civil Surgeons who don't rip you off' that was started by Nik+Heather that should list someone in your area that is cost reasonable and doesn't insist on completing another full exam.

And if you don't have time to read at work, then wait til you're home or ask your fiance to read it and understand what his process is...

Um I would just like to note that you can't do AOS till after you are married. You say you aren't marrying till April so you will do it afer that and as far as the medical goes no he does not need another one. Just take what he has as in vaccinations and print out the I-693 doctor needs to fill in part 1,2 and 5 and on USCIS look up CS in your area as you can only use doctors from the USCIS site for this. My transcribing cost $50 and I didn't even see the guy lol
