I'm a bit confused at the question. If the child has a US passport then they are a citizen and have no need to prove anything otherwise, the child can come and go as they please.

Child can come and go as it pleases. Why was the fiance visa denied? or did it even make it to the interview stage? Having a USC child will usually not play a role in approval of a visa for the mother.

No DNA test will not be needed unless the parent was seeking immigration benefits for the child. You mentioned the child has a US Passport so benefits were claimed prior.
