I wouldnt use separate envelopes for each application. I used big paper clips for each form and then just clipped them all together at the side with a cover letter for all three.

Its been a couple years, since we did our AOS (we're filing ROC now) but I'll try and help a wee bit.

We did a concurrent filing for our AOS process. We put the I-130 with its own cover letter in a manila folder with the pages 2 holed punched at the top and proper clips. Then we placed the I-485, I-864 and I-765 in a second manila folder, 2 hole punched at the top with proper clips. This second manila folder had a separate cover sheet. Now, we also did two separate checks, one for the amount of the I-130 and one for the I-485 plus biometrics fee. These checks were paper clipped to the proper form in each folder.

We then made a cover sheet to place on top of both folders. To hold this on, we used large rubber bands and criss-crossed the rubber bands, with this main cover sheet beneath, so it stayed in place. ( I hope that makes sense) We labled, tabbed, and separated everything into very neat, organized sections, so they could find what they wanted with ease, in each folder. We had no complaints, when we went for our AOS interview.

We also did everything in duplicate (same thing with our ROC process), so that we had two exact copies of the folders we sent out and at home. When we went for our interview, we carried in the second folder, with all originals they didn't ask for, in the same formation, which made it really easy for us to give them anything they asked for, without any hesitation.

Just make sure you explain in your cover sheets for each folder, what you have in those folders, in order by section.  We wish you the best of luck and I hope this helps.

When we did our photos, we were told they had to be in a clear baggies (Ziploc) with pictures clearly labeled on the back and then the baggie was taped in the center of a blank sheet of white paper. I don't believe they are supposed to be in an envelope. Things may have changed, since we did this part though.
