I received a request for evidence in the mail today. So disappointed!

The sheet basically says

- A review of all the applicants USCIS records fails to reveal form ds-2053, ds-3024, ds-3025 OR ds-3026 which are the Depart of State's medical forms.

NEW FORM 1-693: as such the applicant is required to submit a new form i-693, report of medical examination and vaccination record that is properly completed and signed. The newest version of form i-693 includes the medical information as well as the vaccination record. etc...

So what's the deal?? I had my doctors exam in Canada before I got my k1 visa as procedure dictated, on June 9th, 2010. I didn't think i'd have to do anything else medical wise as long as I filed my adjustment of status within a year of this physical/vaccinations were done? I never received a DS-3025 from the doctor. Did the doctor not include this in my medical packet?? I googled it and I have a file that looks basically identical but instead of being an american version, it is a "vaccination documentation medical worksheet two" which says "For use in Canada Only".

What do you think I should do? Call the 1-800# provided with my request for evidence? Call the doctor in Canada? Do the whole thing all over again? I think it cost something like $300 american when I did it last, i'd really rather not pay that AGAIN!

The copy of the vaccination work sheet should of been sent in with your AOS packet. If that is all that is missing. They may be requesting a FULL medical because it kind of sounds like they lost your whole medical.

Regardless of what you did in Canada and regardless that some folks get by without submitting a I-693, many folks get RFEs because they did not. You are one of those. If you follow the VJ Guides you will read that they suggest having the 693 partially filled out, meaning only transcribing the vaccination records form the summary forms you brought with you from Canada. Worst case is a complete medical and add'l vaccinations. Get cracking on finding a local CS (list from USCIS website) and call around and get price quotes. This is not the end of the world and you NEED to provide this for your AOS to continue.

I have one more question; I think from what I read the new i-693 has a vaccination sheet in it. Should I insist when I go to the civil surgeon that they fill out a DS-3025 (I believe that's the vaccination chart) which is what the USCIS said they didn't have? I suppose they'll know what they're doing... just don't want another request of evidence coming back!

Bring the copy of your vaccination records, if you have the one from the medical in Canada still, that might be the easier way to do it. Other wise you need proof of all your vaccinations from something else. They should automatically check your vaccination record and fill one out. If they don't look like they are going to do this, just ask them.

I had the same RFE. They lost my medical. #######!!!

I don't understand this. I see many posts where USCIS lost the medical results. If this happens to me, I would demand USCIS pay for a new medical/immunizations/etc. It is their fault they lost the medical. A K1 Visa cannot be issued without a valid medical exam and that medical exam is good for one year to include the AOS. If USCIS refused to pay for the medical exam, I would speak to or write to the head of USCIS. IF they did not respond, I would immediately file a complaint with my US Congressman/Senator and also write a letter to President Obama. In my letter to the US Congressmen/Senator, I would demand that USCIS pay for the medical.

Has anyone tried this?

They just lost mine too - they tried to blame it on my POE being small, and said I should contact them to see when/where it was sent. I did, got all the details, including the name of the person who signed for it. Called USCIS back; they said to put all that information on the RFE and send it back to them. Keeping my fingers crossed that they 'find' it and I don't have to do it all over again. That's a big expense that I don't need!
