Hi,

So if someone has two jobs....how to they complete the I-134. Its asks for the place of work but only gives a place to put one employer down. Do I need to complete two I-134 forms?

Also at the end the form has a spot next to the part about "I acknowledge and read..." it looks like it wants initials or another signature....?

Last question....it asks how much savings, and I will put that down, but it also asks for life insurance....i have life insurance through my work....do I put how much I have insured myself for?

Either write both jobs on the same line and use the total income from both on the line provided - or attach a separate sheet detailing each job and each income. The most importance for your I-134 will come from your supporting documents (pay stubs, tax transcripts, letters from employers).

If your income is over the requirement, you don't need to list ANYTHING about bank balances or life insurance - they are only meant to supplement low income. Take note that in regards to life insurance the form also asks for its "cash surrender value" - which is only available on permanent life insurance policies. Term life insurance has no cash value at all. Just FYI.

The little blank at the bottom appears to be a glitch in the pdf form creation. Leave it blank. It wasn't in previous editions and makes no sense for any kind of data in my opinion.
