Package 3 is lost - Help with what is in it

I called NVC today and NVC told me that US consulate in Vietnam sent package 3 on February 18 and now it had been a month and we haven't receive it. I suspect it was lost. Any suggestion on what to do? What is exactly in the Package 3 for Ho Chi Minh City? Should I send DS-230? Should my fiancee try to talk to the US Consulate and tell them that our package is lost and possibly the address is incorrect?

Thanks

Thanks! I'll fill out DS-230 Part 1 and DS2001 and will have her submit them at the US consulate in HCM. Neither my fiancee or I received package via email. Perhaps after DS2001, they will send us email attachment. DS2001 has a place where we can put our email address, DS-230 also have place for email addresses.
