Hi Everyone,

I hope someone can help me. My fiancee needs to get her passport. She has NSO Birth Certificate,Voter ID,NBI Clearance and School Form 137. Is this

enough? Does she also need her Baptisimal? Thanks...

Here are the requirements for Philippine passport application. I'm assuming she's applying for her first passport?

2 supporting documents along with her 2 valid ID's is good enough. Sounds like she has more so its all good.

FIRST TIME DFA PASSPORT APPLICANT REQUIREMENTS

* Personal Appearance is required.

* Duly accomplished application form.

* Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO); for born 1950 & below, Negative Birth Record from NSO & Joint Birth Affidavit from Two Disinterested Persons if no record from NSO.

* Three (3) colored photos of the applicant taken against a royal blue background & no shadow. Applicant should be in decent attire with collar & sleeves and in dark-colored shirt. Photo must be studio taken & of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: About 3cm-tip of head to chin; with 8-10mm head clearance from top edge. Ears and forehead should be visible. Wearing of accessories, colored contact lenses or eyeglasses, with name/name board are not allowed. DFA has the right to reject photos that do not comply with specifications and international standards.

* Original ID i.e. Senior Citizen’s ID, Voter’s ID, digitized government-issued IDs like SSS, PRC, BIR, Driver’s License, original school ID (for students only) AND original supporting documents indicating full name, date and place of birth and citizenship (Please refer to the list below)

* If late registered, submit 2 original supporting documents indicating full name, date and place of birth and citizenship prior to date of late registration.

* NBI Clearance (not required if presenting 2 digitized Identification Cards.)

* Certified True Copy (CTC) of BC issued by the Local Civil Registrar if BC secpa is unreadable

* MC secpa of parents if Last & Middle name are foreign sounding (Chua, Ong, etc)

Hi I would like to add my coment to this question ...I live in australia and am going throught hell to get my Philippine girl friend a passport.

She took 7 months to get her Birth Certificate...thats because she moved from the north to the south near manilla 15 years ago, and had to go back up north to find her mother and midwife who was there on the day she was born. She is 29 years old now.

When she got the Birth Cert I gave her money to get a drivers licence, for ID, she got photos IDs and a postal ID. But they would not accept Postal ID as a requirement for a passport. She has no end of year school books so that was out.

She went into the passport office and lined up in the heat with 200 other people and was told no its not enough ID.

I got onto different web sites and have found out in her case that 4 IDs are needed Birth Cert, Drivers Licence, Which I paid for and Bank Account and last but

not least would you believe a Library Card that is memeber of a library. You guessed it no library in SAN PEDRO LAGUNA. She is now trying to get a membership of a school library in Laguna.
