Hi, I am in the process of getting evidence together to submit the I-751 for my wife. I was wondering if someone had a format for the affadavit letter that they would be willing to share.

1) I am making this affirmation in connection with the application of (beneficiary’s name), for a (form), (form name). The contents of the affirmation are based on my own knowledge and are true.

2) I was born on (date) in (city, state, country).

4) (name of applicant), the applicant, is my (relation).

5) Explain how you met. Give dates be specific. Anything to show that the couple in question have enetered marraige in good faith.

6) I have personal knowledge of the bona fide Marriage relationship of (name) and (name). Their marriage is legitimate, viable, and not entered into for any immigration-related purpose.

Doesn't necessarily need to be notarized (mine were all fine with no notary), but if you need extra proof, or feel your application needs to be "beefed up", definitely get it notarized.
