Does it mean my Mothers name at my birth or her own?

My Wife thinks it should be her Maiden name at her Birth, and I think it should her surname at my Birth.

Any idea? lol.

Your mother's maiden name is her surname at her birth....not your birth.....Your wife is correct.

You should put her MAIDEN NAME .. the name she had when she was born... if is / was married.. you but the name she has before any life change event/s. In other words - the name that is on THE MOTHER'S birth certificate.. not the DAUGHTER since that could be different.
