OF-167 is a simple one page form the USC fills out and signs. ( 1 min total time ) IF your consulate requires this it will be in your package 3. Or possibly you can email them and ask as it's consulate specific (Vancouver Canada requires it and Montreal Canada does not *As an example*)

And seems its only for K3/K4 visas.

I did a little more looking on that site and found the 2 page form:

http://usembassy.state.gov/posts/mx2/wwwfof167.pdf

However both pages are the same just page 1 english and page 2 spanish. so I suspect this may vary from consulate to consulate, and the second page refered to may be a questioner presented by some consulates and not others.

Anyway OF-167 should be in packet 3.

Here is what Vancouver consulate sent me in an email ( they do require it but many consulates don't ) This attachment messes up @ the bottom so i didn't use it.. I waited for package 3 and faxed that one to my finace. Anyways .. it does give you the idea

Just to make sure everyone's clear on it, the form Robin28 attached (which is one I filled in when going through Vancouver on a K1), is the OF-167 Attachment. There is apparently also an OF-167 form used for other purposes (which may or may not include the K3 and K4 or other visas).

OF-167 is the instructional document that outlines the information that can be presented to meet the public charge provisions of the law. OF-167 is it's old designation (long before the K3 and K4 visas existed), it is presently designated DS-2000 although some consulates continue to use reproductions of the old form.
