Does it have to be Form 1040, it could be 1040A also right? As long as the tax returns are filed with IRS, the embassy does not care about the forms, correct?

Bring whatever you filed, 1040, 1040A or 1040EZ.

For 2008 and 2009 returns, do I need to sign and date or just a COPY stamp is enough??

I think signed and dated would be a better choice. But a tax transcript would be excellent as well.

For 2008-2009 what date should I put? Obviously since they were filed in 2008 and 2009, the date should be for that time and NOT 2011, correct??
