Hello VJ fellas

Today I went to get my DS - 3025 (vaccination sheet)transferred to I - 693 form. However the nurse in charge, told me that my vaccinations are not current (need follow up vaccinations, since its been almost over 6 months).

I told her I would not get them for now and basically I only needed my vaccination sheet transferred into that form. Therefore she gave me the form, Part 1 filled up by me, Part 2 filled up by them (only vaccination part), and Part 5 signed by the CS but none of the results were marked (below):

- Applicant may be eligible for blanket waiver(s) as indicated above

- Applicant will request on individual waiver based on religious or moral convictions

- Vaccine history complete for each vaccine, all requirements met

- Applicant does not meet immunization requirements?

In the vaccination page only was written, the date I got "Td", "MMR" and "Varicella" shots back when I had to get it done before my K-1 interview.

My question is, should I get the vaccinations completed? Is this required due to the interview? Or do I only need the vaccination sheet transferred?

You need the vaccination sheet transcribed AND you need to be up-to-date.

First, here's the list from USCIS telling you what you need: http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=3384cc5222ff5210VgnVCM100000082ca60aRCRD&vgnextchannel=6abe6d26d17df110VgnVCM1000004718190aRCRD and then here's the table from the CSC (that the CS's use) to determine how many of what shots you need and at what age range: http://www.cdc.gov/vaccines/recs/schedules/downloads/adult/mmwr-adult-schedule.pdf page 2. You'll see varicella is a 2 dose shot. If you only had one then you need the second IF the time for them is due. MMR is 1 or 2 doses and normally they'll give you 2.

When I say "the time for them is due" it means some shots you need to wait a month, or two, or 6 or 12 between doses which is what I think the CS was saying by you need your shots updated.. you might be at the time for second shots.

Again Vanessa thank you so much for your quick reply,

According to the guides you linked me to, I will be needing a second shot of Varicella, second shot MMR (if the CS thinks I might need it) Also a first shot of HPV since i am younger than 26. She also stated I need the Influenza shot, but I got that one back in October so I'll be heading to my local pharmacy to get my records.

So far, for my medical exam before my interview, they gave me the "Td", "MMR" and "Varicella" shots on 09/13/2010.

I don't have records of other vaccines :bonk:

PD: I wasn't able to see your shot record, the file does not exist.

You don't need HPV. That was thrown out in late 2009. Not 100% on the date but 100% know it's no longer required.

Without results marked, they will probably reject the I-693 if you send it in like it is. Many, many people are successful with sending in only a copy of the DS-3025 if you have MMR, Td, and Varicella marked AND the results section marked as Incomplete...eligible for blanket waiver, and signed.

USCIS would go by the date of your medical exam and you were current at that time. Only starting the series is acceptable. They don't make you wait for the months intervals before you can finish your process. By going to a civil surgeon for the I-693, you have a created new "date." I think the nurse was accurate by saying you were due your second of series because time had passed, so to get the CS to sign off, you may be forced to get them. BUT it's my opinion, had you not gone to a civil surgeon, the USCIS would have accepted your DS-3025 if it was marked as I said. The only question is how they would treat the flu shot. If your DS-3025 was completed when it was not flu season, then you would not be required to have one. If your medical was April-Sept, then you're okay. By resetting your date with an I-693, then that would become the date and document USCIS looks at, and because it is now flu season, you would need the shot.

Briefly, it's my opinion that your DS-3025 would get you through if you sent it instead of the I-693. If you go with the I-693 then you need more shots and the results section completed.

I just wanted to share with you guys, that I decided to get my pending shots at the AHHSD Health Dept, it costed us 10 dlls. Then I headed back to the Health Department Center where I went initially to get my vaccination sheet transfer, which I paid 29 dlls for it.

All in all was 39 dlls, plus 10 dlls for the flu shot ( got this one last year)

Now I am all sore but it's something less to worry about. :D
