I'm filling out the I-134 Affidavit of Support and I only intend to provide the following since my income is above the 125% poverty line.

SIMPLE Tax transcripts from the IRS for past 3 years

W2 for current year.

Letter from my employer stating annual salary, job responsibility, and that is full time, on company letterhead.

Photo copies of past month or so of pay stubs

My question is, should I only provide a value for "I derive an annual income of:" since that is the only evidence I will be providing?

Or do I still need to provide values, if applicable, to the other sections such as stocks, real estate, mortgage, etc. if I will not be sending evidence?

If you are above 125% of the poverty threshold and you provide the adequate proof such as you listed, the other lines are unneccessary. They would only be required if you are trying to show ability to provide support based on assets.

You might want to do 2 to 3 months of current pay stubs instead of just 1.
