As what the title says, does my son need to have his medical exam too? Im going to be a K1 applicant and we included him in our petition as K2.

You will need to make sure his vaccinations are all current and that he has what is needed for immigration(usually cheaper if done by your own personal doctor than by the physician listed for the medical). He will have a medical but he will not have to have an xray and he will not have to have blood taken.

You don't "have to" as no vaccinations are required to obtain a K visa. However, they will all be needed to adjust status after US entry and marriage, so it is both less expensive and more convenient to get them done in advance for both of you.
