From searching the forum for this answer it seems that since my non-US citizen husband completed his medical exam before his interview when he was still abroad, that we do not need to fill out and turn in the I-693. I have also searched for the DS-3025 in which I am unsure if we received. My husband did however receive a white envelope from the doctor when he had his medical exam abroad that has a stamp on it that says "DO NOT OPEN" as well as 2 printed stickers (one of front and one on back) that state "K1 - applicant copies of medical forms. Retain this sealed envelope for your I-485 application with the USCIS Office. DO NOT OPEN". I assume this includes things about his vaccinations and exam to be used as 'proof' of his records. So the 2 questions I have are:

Has anyone else recieved anything like this?

Would this contain the DS-3025?

It should. That's nice of your doctor to do that for you. I have not heard of that before.

My husband and I are having the same delema right now. He showed me his white envelope yesterday. It has no sticker on it about not opening and it is held together by a flimsy piece of scotch tape.

This envelope should be included with the AOS packet? DO you think it will be a problem that it's only held close with a flimsy piece of tape? We have not opened it.

I read the links that Vanessa & Tony posted from past posts about this envelope. I hate the fact that it cannot be opened because we cannot get copies of whats inside if they misplace it. I think based on what I read, that we will be including this in the package, by the I-485.

I had an envelope like that from my medical in Frankfurt/ Germany. I included it in the I485 application, and did not do another medical here in the US. My application is still pending, so I don't know if I was right. So far now RFE though.

I just got mine done few days ago, its the I-693 paperwork they had me check if my info is correct like my name address etc. then they filled out the rest of the form from my lab test vaccine, tb test etc. they put it in a sealed envolope told me not to open it and gave me a copy of whats inside it, you can ask your doctor for a copy of your I-693.
