HELP WITH MY I-130

Hey everyone im new to the site and need some help. I brought my filipino wife to the usa on a spouse visa and she came to the usa and we got married and after 5 months she went back to the philippines. The reason she left was the fact she had a hard time adjusting to the culture and the fact that i am an over the road truck driver and gone all the time. Now we have been married for almost 4 years we have a condo in the philippines where i stay with with her a month or so a year with her. I want to bring her back to the usa due to the fact she has realized she should of never left and im haveing a problem on how to fill certain answers out on the i-130 form. the question about what was the last address we lived togethere, do i use the one in the philippines or when she was in the usa , and the question about if she has been under immigration before but reason she left isnt on there. any help is appreciated.

it was a fiancee visa we married in the usa when she arrived
