Tourist visa to China

My wife and our 22 month old daughter plan on visiting China in April. I could use some advice and perhaps the best agency to go through for the following:

My wife has her 10 year greencard and a current Chinese passport.

Our 22 month old daughter (born in the United States and of course a U.S. citizen) has a U.S. passport.

What is required for both my wife and my daughter to visit China for a month and to return back to the United States once their visit is over.

I'm in the process of completing the "Visa Application Form of the People's Republic of China". Section 2. Your visit to China, block #2.2 only goes up to "Multi-entry valid for 12 months". Where do I specify on the form that I want a 2 year multiple entry visa for my daughter? Is this the form that I should be using or is there a different form for the 2 year multiple entry visa?

Yes, it is the form. On that box, you just fill in " Multi-entry valid for 24 months", which is stated on China Consulate's website. We did like this.

Keith - that form is a bit outdated, there's a newer one floating around somewhere -

but

you can make new tick box for 2 year, draw it in, write legibly.
