was getting ready to prepare fourms i need to file and was wondering do i put her new last name? her SSN has my last name her state id now has my last name so do i fill this out with new name? thanks in advance

Ok so you are doing AOS yes? You need to put her new name which is her married name on everything.

Divorced !st November 2012.

Married only 2 years 1 month

File with the last name that has on ID, passport and SSn

This is incorrect.

OP - file with the name that she will be using from now on. So, yes, the "new" last name. It is okay to have old passport still in maiden name. Change it eventually, if you can (some countries have different rules on name changes). Good luck.

Yes, if this is the name wished to be on green-card. There is a line on G-325A "other names used" this is for maiden name...

thanks for the info one other question on the i485 do i use the passport number or the control number?

Visa number.

Forms asking for I-94 # use the pre-printed number on the I-94 card stapled in passport.

A# can be found on I-129F NOA2 letter for Fiancee visas.
