question #2 place of issuance where would you find that info out?

also #30 have you ever been issued a us visa? it says where, does that mean i would put the airport?

i dont know im so confused :wacko:

April 25 2006 ~ sent I-129

May 4 2006 ~ NOA1

July 31 2006 ~ NOA2 APPROVED!

Aug 24 2006 ~ recieved package 3

Aug 25 2006 ~ faxed D-230 to consulate

Aug 29 2006 ~ faxed checklist to consulate

Aug 30 2006 ~ attended medical appointment

Sept 1 2006~ recieved interview date

Sept 12 2006 ~ Interview... got visa!

Sept 13 2006 ~ got on plane to the usa

Place of issuance, is where you recieved your passport. Should be on the photo page of the passport.

You, as a Canadian Citizen do not require a US visa for visits. So unless you worked in the US or something, enter "none" on that line.

Question #3 -- issuing country -- refers to Question #1 - the passport of the beneficiary. Therefore, #3 means - where was the passport issue i.e. city, province, country, etc.

#30 means have you ever been issued a VISA i.e. student visa, work visa, non-immigrant visa. If yes,

answer when (year) where (country) and what type.

Unless you are preparing for a consulate interview post I-129F approval, you do not need this form yet.
