civil surgeons - chicagoland area

hello,

anybody who knows or have had a good experience re the above? i've checked a few using UCSIS locator and they seem to charge exorbitant fees and want to do the full medical.

any info is appreciated.

aAOS process:

Filed AOS, EAD ,AP - Aug 19, 2005

Received Notice of Receipt - Aug 23, 2005

AP approval via email - Nov 1 touched, Nov 2 and Nov 3, 2005

AP in the mail- Nov 08, 2005

Infopass appt - Nov 25

-- informed that my EAD has already been approved!!; given appt for biometrics.

Biometrics for EAD - Nov 25, 2005

EAD email approval - Nov 29, 2005

EAD card in the mail - Nov 30, 2005

---- validity started Nov 01; means they've approved it as early as Nov 01 and further processing was stalled bec i have not done my biotmetrics ( well, how could i? they told me to wait for an appt letter from them! ) anyways, job search starts for me now.

Biometrics - April 2006

AOS Interview - Oct 2, 2006 - APPROVED

2 yr GC received 2 wks later.

751 process

Filed papers - mid Sept 2008.

Biometrics - Nov 2008

10 yrs GC received - Dec 23, 2008

Citizenhip process

Filed papers - March 26, 2010

Biometrics - late May 2010

Interview - July 14, 2010

I went to one on W. Peterson and paid $150 i think for everything. Didn't take too long either. I called to a few places and the price varied from $150 to about $250 or so.

I can forward you the details when i get home tonight if you want to go to that office.

I am not sure how far Rockford is away from the Northwest Suburbs but it might be worth the trip as all they charged me to do only the suppliment was $10.00 ... I brought my form I got from the doctor in Canada and also the forms I got from my doctor showing the shots I had ...

The name and address of the one I went to was:

Physician's Immediate Care

I talked to a few people at the clinic and most of them didn't know what I was talking about so eventually I talked to one of the nurses named Jason and was relieved when he knew exactly what I wanted and he said he would call my back to let me know how much it was once he talked to his superior ... He called me back in a few hours and told me it would be $10.00 ... I also overheard one of the other nurses while I was there talking to a superior on the phone and she was wondering why they had to do this paperwork if they didn't do the medical exam ... Anyways after an hour wait everything was sorted out and I walked out of there with a seal envelope only costing me $10.00 ...

You don't need an appointment, just drop in ... If you call and check I believe they are open to 7pm on most days ...
